It works like this: In lieu of a seat, many airlines offer a bassinet that bolts to the wall of the bulkhead (the seats at the front of the plane). There isn't usually an additional charge for this seating option, but space is obviously limited, and typically, there are just two bassinets available per flight, so it's first come, first served.
